2026 - 2027 Back to School Packets Forms and Information
Welcome to the Encinitas Union School District’s Digital Back-To-School Packet, which is accessible through your Parent Portal starting on August 10th. All the information can be accessed on the EUSD website at Back-to-School Packets. If you do not have access to internet at home, or on your phone, you can visit your school site or any public library to complete the packet. If you need assistance, please contact your school office staff.

FIRST (BEFORE FILLING OUT FORMS) - Read the Required Information

NEXT - Access Back-To-School Required Forms Through Parent Portal:
1. Log in to your Parent Portal here, or create a Parent Portal account if you are new to the District. (Click here for account creation Instructions)
2. Once you have created your account, select the student (if more than one child).
3. Choose: Forms – Student’s current information is pre-populated (demographics, contacts, etc.) Make changes or keep the information the same. Remember to hit “Submit”.
Complete all forms including:
a. Emergency Information Card
b. Mandatory Signature Form
c. Medical Form
d. School Funding Form - This form helps to ensure that adequate funding is available to support educational programs that directly benefit your child. Please complete the School Funding Form to ensure your child’s school receives the funding it deserves.
e. PTA Student & Family Participation, Volunteer, and Indemnity Agreement waiver (Filled out through the PTA Link provided in the portal).
