2024 - 2025 Back to School Packets Forms and Information

Welcome to the Encinitas Union School District’s New Digital Back-To-School Packet accessible through your Parent Portal. All the information can be accessed on the EUSD website at Back-to-School PacketsIf you do not have access to internet at home, or on your phone, you can visit your school site or any public library to complete the packet. If you need assistance, please contact your school office staff.

NEXT - Access Back-To-School Required Forms Through Parent Portal:

1. Create a Parent Portal - Click here for Instructions

2. Once you have created your account, select the student (if more than one child).

3. Choose: Forms – Student’s current information is pre-populated (demographics, contacts,

etc.) Make changes or keep the information the same. Remember to hit “Submit”.

Complete all forms included:

a. Emergency Information Card

b. Medical Form

c. Mandatory Signature Form

d. School Funding Form - This form helps to ensure that adequate funding is available

to support educational programs that directly benefit your child. Please complete the

School Funding Form to ensure your child’s school receives the funding it deserves.

e. Volunteer Code of Conduct

f. PTA Parent’s Approval, Student Participant Waiver

g. PTA Adult Participation/Volunteer Waiver, Release and Indemnity Agreement
