The following information is necessary at the time of kindergarten registration:
- Proof of residency within the school attendance area.
- Birth Certificate - Original and one (1) copy (we keep the copy). A child must be five (5) years of age on or before December 2, 2010 in order to enter kindergarten.
- Two (2) emergency names and phone numbers - other than custodial parents or anyone who lives at the same address with the same telephone number.
- State Immunization Card - Children are required to have the following immunizations for entry into kindergarten:

4 | Doses at any age, but three (3) doses meet requirement for ages 4-6 if at least one was given on or after the 4th birthday. |
DPT |
5 | Doses at any age but four (4) doses meet requirement for ages 4-6 if at least one was on or after the 4th birthday. |
MMR |
2 | Doses required for kindergarten entry within 4 days of age 1 or after. |
Hepatitis B |
3 | Doses required. |
Varicella |
1 | Dose required or documented proof of chicken pox. |
If your child's immunizations are incomplete but current, the child can be registered. The child's immunizations must continue on the recommended timeline until completed. If your child's immunization record is missing required doses, please contact your doctor or clinic now. All inoculations are available at your physician's office, or they may be obtained for a small fee at a public health clinic.
