2009-2010 Kindergarten Registration Packet — Forms and Information
The “Required” information is necessary for every returning student. You can type/fill in your information, print the form, sign the form and return to your school on the required return date.

Required
Birth Certificate — Original and one (1) copy (we keep the copy). A child must be five (5) years of age on or before December 2, 2009 in order to enter kindergarten.
Two (2) emergency names and phone numbers — other than custodial parents or anyone who lives at the same address with the same telephone number.
State Immunization Card — Children are required to have the following immunizations for entry into kindergarten:
